Langstone Way Surgery
Your health is our priority!

How to register with the practice

  • Patients register with a practice but will still be allocated to a particular named Doctor. This does not mean that you may only see this doctor. If you are requested to attend for a face to face appointment, you will see whichever doctor is available. Alternatively, you can always ask to see the Doctor you prefer, but should understand that this may not always be possible.
  • You must also complete the Welcome Questionnaire. by clicking here. Please don't forget to do this. It will help your doctor to manage your health problems as a new patient, until the surgery receives your notes from your previous surgery. Thank you.
  • Registration is normally undertaken between 10:30am and 5pm.

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
